What Causes Red Itchy Spot Near Anus?

I have this round red spot near my anus gets very itchy then it drys up and the skin flakes the itch stops the prosses starts every few days all over again.I use this cream hyderm 1%cr+nystat cr (wmt) Could youu tell me if I shoul keep using or go back to the doc. Thanks
