What Causes Redness At The Urethral Opening Of A Child?

we have seen our pediatrician. Our2 year old boy s urethral opening is red. we have tried (and been directed to try) lots of aquafor as a buffer incase it is chaffing, monistat 7 day (doctors direction) and nothing seems to be working. He does not seem to be in pain no discomfort washes it fine at bath time. just a little concerning, not getting better, Its also not getting worse.
