What Causes Scab On Head?

7 yr old daughter has large isolated scab on head. Had 2 lots antibiotics plus special shampoo. It's not worked. Been told it isn't ringworm or dermititus but it is itchy and she has had it 2 months now. Could has evolved from a bite because she remembers a lump where it first itched.
