What Causes Scaly Black Hard Nipples And White Rash Around Them?

hi. i saw a dermitologist a year back and they said i had nipple eczema.its hasnt cleared up yet so wanted a second opinion.my nipples have turned black in places and have become hard cracked and scaley.i can peel the black stuff off but it leaves a crater like hole in nipple untill it grows back and turnes black again.also i have a white rash around my right nipple.thanks
