What Causes Severe Constipation Along With Bleeding In Rectum?

I am constipated most of the time. If I take a laxative, only small, hard stools appear. I have bright red bleeding lately around rectum ., small amount. I have had polyps removed -also i have difficulty digestingfoods. I am 80 yrs.old. and weigh 158lbs.
