What Causes Severe Headache Post Sinus Passage Correction?

I've been plagued with sinus/ear infections most of my life. I've recently connected the blocked sinus passages to my eustachian tube. When driving, I have to crack the windows. Flying...take Afrin. My problem is that every night after lying down, I wake with blocked sinus/ear passages and have a horrible headache that throbs in my right ear, also. Why is this? And, what can I do to remedy it? I've had the tube put in my ear. Of course, it didn't last. I'm almost 50 and am becoming nervous about still suffering from these horrible headaches as they keep me in bed.
