What Causes Severe Headache,ringing In Ears And Chest Pain?

Hi Doc,Trying to help my 48 year old brother. For the past week he has been experiencing severe headaches, ringing in his ears and high levels of pain where his shirt touches his chest, underarms as it the pit area and under biceps. No rash, bumps or skin discoloration. Went to an urgent care center three days ago and received a steroid shot which dropped the skin about 30% for about a day and then returned. Had a temp and body aches for about a day.Any thoughts?
