What Causes Severe Migraine With Changes In Pupil Size?

My husband gets chronic migraines usually over his right eye upon waking. Thus time is different though he woke up Monday morning with a severe migraine over his left eye he said feels like an ice pick driven through his left eye. Imitrex hasn't helped. It hasn't gotten better and now his pupils are different sizes. Is this something to be really concerned about?
