What Causes Severe Pain During Pap Smear?

I,ve had extremely painful pap smears lately, she had to stop because it hurt so badly that I cried out loud! She didnt seem to understand, she said she had used the smallest forceps she had. I'm sixty yrs old, am not sexually active, and have had a hysterectomy approx. 15 yrs ago. Also recently I inserted a finger inside my vagina and it felt so tight like the walls of my vagina had cloed up. ?????
