What Causes Severe Pain In Hand And Shoulder?

sir i have a rhematoid problem in past two yrs. but in blood test i have negative in rheumatoid hsr is high. in hand and shoulder portions have a small black marks there. at the moment i have a severe pain . in lips also that problem. and i had a fever also. pls tell me the solution sir.
