What Causes Severe Pain In Shoulders And Upper Arms?

I have been have severe pain in my shoulders and upper arms. At this time the left side is worse. I went to an orthopedist who did x-rays and other tests and told me I had carpal tunnel and did surgery on my right hand. That did nothing to help my shoulders. I know from the x-rays it is not a rotary cuff problem. I don't know if I should find another orthopedist of if I may need another type of specialist.
The pain has been so bad at times that I can't function. Movement does not seem to have any thing to do with it. It just hurts all the time.
