What Causes Severe Pain In The Back Followed By Bleeding?

I have had some thinning of my hair, my lower back seems like its under my ribs on both sides of my lower back are painful, as i try to stop peeing it is very painful with alot of pressure and I have had some bleeding. Also I have had an iud marina for 3 yrs. What could this mean?
