What Causes Severe Pain On The Top Of The Feet?

Hello my husband is a marathon runner and has been experiencing a throbbing pain on the top of both feet. This last marathon his pain became sharp and there is also some minor swelling on top as well. Just wondering if this seems serious are if there are things that he an do besides icing that will heal this problem?
