What Causes Severe Pain While Removing Tampons?

I was wondering what causes extreme pain while removing tampons... I have used tampons for years but occasionally when I remove then the pain is so bad it feels like in trying to pull out my insides... And once it s out tha pain continues... This doesn t always occur only sometimes when tampon is fully saturated
