What Causes Severe Skin Itching In An Elderly Diabetic Female?

My nani has got some type of skin infection in which she scratches her skin very badly to the point that it bleeds, her skin has become black and she says that the skin burns which causes her to scratch her skin. She is a sugar patient. I feel really bad for her when she gets up in the night due to pain. Can you help?
