What Causes Severe Tightness In Neck And Shoulders After Taking Dilaudid?

I was prescribed dilaudid 2mg every 4/hrs as needed for pain bc I just had surgery but when I take it I get a severe tightness in my neck and shoulders and can barely turn my head. In the ER when they give it to me by IV I don t feel like this. Why the stiffness now?
