What Causes Severely Strong Foul Smelling Urine?

Hi I work with seniors, One of our male seniors has a problem, when he goes to the bathroom to pee he smells very bad. the smell is so strong it comes out the bathroom and in the hallway. Its also stay with him the rest of the day. Its smell like a strong gas.
What is it and what can he do?
