What Causes Sharp Pain In Nipple?

I am 47 and for the last couple of days i have a sharp pain in my left nipple. this morning i noticed a red spot just below the nipple and also a little red place on the nipple that looks almost like a crack. i have put some antibiotic cream on it but it didn't seem dry and i haven't had an injury...should i be concerned?
