What Causes Shortness Of Breath Along With Dizzy Spells And Light Headedness?

Im 28 yr old female. My maternal grandma had heart problems and never smoked or drank. She had tripple by passes a stroke too. Ive been smoking since I was 12. Im active but with in a few months now, notice shortness of breath while doing normal activities, been tired, hard to sleep and have been waking up in middle of night which never happens, also been feeling dizzy and lightheaded, even when I wake up. Have been feeling numbness on left and right arms and hands all of theese symptoms I have never had before. I dont have health insurance, everyone says im fine. What should I do.
