What Causes Shortness Of Breath And Chest Pain Upon Exertion?

My wife get short of breath and experiences chest pain upon exercion. She had a Heart Cath Angiograme which showed no blockage. Also had CT Scan, Blood tests....all from a Cardiologist. Yet the pain and shortness of breath remains the same. What is it?
