What Causes Shortness Of Breath, Hearing Loss And Blurred Vision?

Hello. I am 24 years old, daily smoker, social drinker who takes adderal and lexapro daily. I have been having shortness of breath; left sided weakness/numbness/tingling, hearing loss, and blurred vision; extreme exhaustion and "fuzzy" mindedness; as well as chest, neck and shoulder discomfort and sometimes sharp pains. I am an ultrasonographer and thought to scan my carotid arteries. I found a dissection that extends from the right subclavian up the right carotid almost to the bifurcation of the internal and externals. There is also clot formed proximally that is causing stenosis around 50% according to the velocities I got. My question is if this is dangerous enough that I should seek immediate attention or should I follow up with my primary care in the morning? I am still in pain, and the shortness of breath and numbness has been persistent over the past few hours.
