What Causes Skin Rashes With Lumps?

My skin always has rashes or skin eruption everytime i'm scratching it or whenever somebody or i accidentally hit myself it creates lumps. After scratching or hitting, i feel sudden heat on that part of my skin followed by itchiness before the skin eruption or lumps appear. What kind of skin sickness is that and what is the best medication?
