What Causes Slight Bleeding From The Nipple?

Hi, my son is 19 months old. I just noticed 2 drops of blood on his tshirt near his right nipple. Otherwise his nipple looks fine as in no redness or soreness or lumps. It doesnt look like any trauma to his nipple as well. I am very concerned about the bleeding. Please advice.
