What Causes Slightly Flaky Skin On Head Of Penis?

Hi im in a long term relationship at the moment and have regular sex with the same person and have for about a year now, the head of my penis has recently becomes slightly flakey or the top layer of skin is able to be rubbed off, also there are very feint white although you can hardly see them, please help I don't know what it could be and I'm quite worried
