What Causes Smegma Accumulation And Pain Under Penis Head?

I have a large build up of 'smegma'. I'm not circumcised and the skin that covers my penis is itchy. When I pull my foreskin back i get a little bit of pain under my penis head, where the skin is connected to my head. I need help. I'm worried it may be an STD. Please help!
