What Causes Sore And Sensitive Breasts?

My breasts have been sore and sensitive for a couple of weeks now mainly around my nipple area. Sometimes my nipples will look full and as of theyr not there but when they are seen and the skin goes hard it's as though it's filled with nothin but air and as if there is a hole under the skin. Also my nipple has a slit on it as if it is opened. What it this?
