What Causes Sore, Torn And Cut Penis With Clear Discharge?

I have been out of town (iraq for 6 months) i have had no sexual relations. I came home a week ago and have had sexual relations with the same person numerous times. My penis is very sore, around the head of my penis it appears that there are sores/tears/cuts just under the head of my penis. very sore to touch and appears to have a clear discharge (healing)? also my entire shaft is very dry, like dry skin discolored? Is this a std? or just sore?
