What Causes Soreness And Frequent Cuts On Foreskin After Sex?

Hi for a while now i have been experiencing a sore foreskin, i had sex this morning and have noticed a small cut on the top of my foreskin and it has swollen a little bit....i can still retract the foreskin and can see the small cut. eventually this will heal as it has in the past but it continues to happen 1 in every 3 times i have sex.
