What Causes Stringy And Clotty Menstrual Discharge?

I currantly have a pretty heavy menstrual dsicharge that is stringy and clotty. I also have a dull to sharp crampy pain which seems to be originating from bladder or uteris to lower left side about where my ovaries might be. There is a chance I have a bladder and/or kidney infection deduced from abnormal UA. It shows a somewhat higher leukocyte and protein level. I have had the heavy painful menstruation before but it seems this time the discomfort has increase. I am also nauseous, somewhat confused, with dull headache. And usually the pain starts 2 weeks prior to period and abdomin looks abnormally distended and feels firmer and somewhat tender on the left side. I also have pain near the last left rib extending around to back and pain near sacrum that can only be relieved by bending forward or lying down. I'm thinking that I have a bladder, kidney, and some sort of gynecological isssue going on but I'd like to start sorting it all out. So what do think all thise means and where cand I start?
