What Causes Swollen And Tender Nipples Along With Breast Discharge?

Hi, I have recently noticed my nipples are swollen and tender and with a little pressure leak cloudy fluid. I have the implant and a 3yr old. I have been experiencing some aching in my lower back too. I was just wondering if you had any idea what could be causing this. Thank you.
