What Causes Tear And Bleeding At The Site Of Episiotomy?

I gave birth to my daughter 10 months ago, and during delivery, I had an episiotomy. I don't really know anything about it other than that they stitched me up afterwards. My husband and I started having sex about 2 1/2 to 3 months after I had our daughter. Ever since then, I have had slight tearing at the episiotomy site and a small amount of blood from the tear every time. The pain feels like that of a paper cut. I notice it during the initial penetration, and then after we finish. Once we are completely done having sex, the torn area burns for about an hour before feeling better, but hurts for 1-3 days when I urinate. Is there anything that I can do or have done short of surgery to correct the problem?
