What Causes The Tip Of Penis To Bulge While Urinating?

I'm a married male of 40 age,with two sons,the tip of my penis is not open ,even the skin on my tip dose'nt move backward.is it a problem,what difference will it make if the tip opens.While urinating there is a bulge at the tip first then the urine comes out.
