What Causes Tightness In My Chest And Throat?

I frequently have tightness in my chest and throat. sometimes this could be defined as pain. I have had two episodes of crushing pain in my chest that was not accompanied by diaphoresis, pain in my L arm or SOB. I have a family Hx of heart disease and stroke. I am a menopausal 53 yo female , 15-20 lbs overweight (5'5'' 156 lbs) I exercise fairly regularly and vigorously and don't experience lightheadedness or dizziness. My lipids have been elevated for quite some time (over a decade). Finally i am on Simvastin 20 mg q hs and they are perfect now. I am also hypothyroid on a dose of Levothyroxine 125 mcg e am and have been borderline diabetic with symptoms. I have started on Metforman 500 mg 2Xd with excellent results.

Should I get a cardiac workup for this hx?
