What Causes Tightness In Stomach And Vaginal Spotting While On IUD?

I have an IUD that was put in in August 2014. I dont have my periods but when I use the bathroom there is blood in the toilet and also when I wipe. I feel tightening in my stomach. I took 2 pregnancy test both were negative, what should be my next step.
