What Causes Tingling In Finger Along With Painful Neck And Shoulder?

i am experiencing extreme pain in neck and right shoulder. i can cough and my right shoulder feels like its about to come out of the socket. there is also pain down my right arm and tingling in the "pointer" finger and the "birdie" finger. can you tell me what this might be?
