What Causes Tissue Like Growth On Penis Hole?

Hi, i have a penis problem. On the tip of the penis, just on the head, i have a rather like tissue which is growing around the hole(from which i urinate) and it also is attached to the foreskin. When i feel excited, the head cannot get outside the foreskin. i want to know if this is an infection??
