What Causes Twitching Spams In Eye Along With A Bruise On The Back?

My fiance has a quarter sized bruise on his upper right back about 4 inches from his armpit. He said that it does not hurt at all and cannot think of why it would. He has been stressing a lot lately, his right eye has developed a twitching spasm; and if it helps, he has been gaining weight noticeably in his abdominal and hips area for the past 6 months. Do you have any suggestions of what it could be?
