What Causes Vaginal Bleeding After Intercourse?

Hello. My husband and I have been trying to conceive for 3 months now. We had intercourse around my ovulation day this month and then again 4 days after that. when I wanted to clean myself after last intercourse, I saw a blood clot. What does this mean? Does it mean that there could have been conception but it failed? Please help!
