What Causes Vaginal Bleeding And Itching?

I have spot bleeding from my clit for about 9 months. I get a small stain on underwear. Also have burning pain and sensitivity. I went to ob/gyn 9 mo ago C/O itching and burning. Then last night while sleeping my nose bleed a large amt. I woke up to large baseball size bloody spots on the pillow. I have a hx. of breast ca and had a bilateral mastectomy 10 years ago. Did not have lymph involvement and did not have radiation or chemo. I am 61 year old. My hubby had illness and there was no sex for 3 years. He is better now and we have resumed relations. The only meds I take is amytriptaline and tramadol.
