What Causes Vaginal Bleeding During Urination?

Everytime i urinate i bleed .. i wore a tampon assuming it was my period but nothing was there but as soon as i had to urinate again blood started to come down. Then more blood when i urinated. This has been going on for almost two weeks . What could it be??
