What Causes Vaginal Bleeding Post Hysterectomy?

My wife has had a complete hysterectomy about a year and a half ago. She has started bleeding lately. Very similar to menstruation. She is planning on going to her GYN for an annual check-up. Will making an appointment which may take a month or so to get in suffice, or should she go to the ER?
