What Causes Vaginal Bleeding?

I am a 30yr old woman and i use meth on regular basis (not something im proud of) and when i have sex with fiance i always bleed.this has been happening for about four month now and blood clots will be passed from vagina. we have to stop because the bleeding is so bad.
