What Causes Vaginal Leakage Of Blood Clots While On Tampons?

this rarely happens but.....when i have a regular period flow, i get blood clots that seems to come out while i still have a tampon in. but when i pull it out, thinking that maybe its full and needs to be changed.....it barely has any blood on it, yet.....a clot just slid out while a tampon was in. why? do i have another hole down there that i dont know of?
