What Causes Vaginal Spotting After Foreplay?

I had a hestorertomy in September. It has not happened until this month, me and my husband had foreplay and sex. He used his fingure during foreplay. He has very long fingers and I think he might have scratch me inside. I spotted after with discharge. That was earlier this month in January and we where entament again last night an again this time I wiped bleed. Need to know do I need to worry or is everything still sensitive? I and a little sore as well.
