What Causes White Patches On Penis And Problem In Moving The Foreskin Back?

Hi i m 28 Years old & got recently married.my foreskin doesn't at all move back.moreover i got some white patches on my fore skin and some times i feel itching sensation as well.and now the skin is covering my penis completely and i am facing difficulty in passing urine as well.please suggest me.
