What Causes Yellowish Discharge Out Of Nipples After A Vaginal Hysterectomy?

I am 36 has a vaginal hysterectomy a year ago...I am having a clear to yellowish discharge out of my ni[pples upon stimulation...I lost 67 lbs in the last year and have started gaining weight back...I seem to be swelling...at night i am not losing any weight I am actually gaining...HELP!
