What Could Be The Causes Of Sever Pain And Swelling In Ankle?

My granddaughter of 10 has sever pain in her left ankle. She has had all the tests and they have all come back negative. Now they feel that she may be putting it on but I have seen the swelling and her pain. What could be the cause of this as it has been going on for over 3 weeks. Thank you .
