What Could Cause A Rough Patch With Small Tears On The Upper Tip Of My Penis?

Hello! Not too long ago, I started to notice that the upper half of the tip of my penis was a rough patch and was not smooth like the rest of my tip. When erect, I can see that it almost looks like it has small tears and is not smooth. I am concerned on what exactly is going on here!
