What Could Cause Chronic Black And White Patches On The Both Corners Of The Mouth And Chin?

Hii sir, since one year i have developed some black and white patches on both corners of mouth and chin its become dry without applying any moisturiser and it spreads while using creams given by doctor...it look so bad on face .. suggest me some medicine to cure it
