What Could Cause Heavy Menstrual Bleeding For 5 Weeks With Cramps And Back Pain Despite Tranexamic Acid And Progesterone Treatment?

Hi there, I have been having a heavy period for almost 5 weeks. I was prescribed tranexamic acid for 3 days which didn’t stop the bleeding. A couple days after that I was given a 10 day prescription for Progestrone which I still had spotting while I took it. And the bleeding came back heavy again the day after my prescription finished. I waited 3 weeks before I went to the doctor for the first time. I have been having bad back pain and painful cramps. Pregnancy tests also came back negative. Any help or answers would be great. I am waiting for my ultrasound appointment in a couple days to hopefully answer more questions.
